The purpose of this program is to identify residents who may be at risk for re-hospitalization secondary to pulmonary conditions such as COPD and pneumonia. These residents may benefit from a comprehensive pulmonary program including education, optimization of medical therapy, exercise training, psychosocial support, controlled breathing techniques, and continuing care programs in order to reduce the effects of acute and chronic pulmonary symptoms. Physical and Occupational therapists can assist residents to understand the effects of chronic pulmonary conditions, provide instruction in controlled breathing techniques, and provide education related to activity pacing, task segmentation, and exercise programs within the facility and upon return to the home environment. Additionally, Physical and Occupational therapists can serve as part of the IDT team to identify root causes of pulmonary signs and symptoms and make recommendations for overall resident care.
