The purpose of this program is to identify residents who may be at risk for re-hospitalization secondary to cardiac conditions such as heart failure and other cardiovascular diseases. These residents may benefit from a comprehensive cardiac program including education, optimization of medical therapy, exercise training, psychosocial support, and continuing care programs in order to reduce the effects of acute and chronic cardiac symptoms. Physical and Occupational therapists can assist residents to understand the effects of cardiac conditions, provide an individualized exercise program, and provide education related to healthy lifestyle choices and exercise programs within the facility and upon return to the home environment. Additionally, Physical and Occupational therapists can serve as part of the IDT team to identify root causes of cardiac signs and symptoms and make recommendations for overall resident care.
