The purpose of this program is to identify individuals who have had a change in or new onset of cognitive loss. Cognitive loss can have a significant impact on an individual’s quality of life. It may impact their ability to engage in occupations, safely walk and transfer unaided or with limited assistance, communicate their needs, and safely swallow food and liquid. Cognitive loss places the individual at risk for falls, dehydration, negative behaviors, and dependence on others for basic needs. This program is comprehensive and utilizes an interprofessional approach to design and treat an individual’s cognitive needs to promote enhanced quality of life.
